Cancellation Policy
PARENT'S CONSENT AND WAIVER (1) I hereby give my consent for my child to participate in basketball skills training (and future sessions if applicable). (2) I understand basketball is a physical activity and can be dangerous. (3) I declare my child is in good physical condition and is able to meet these physical demands. (4) I hereby give the staff and volunteers permission to render such medical and hospital care as, in their judgement, may seem advisable for my child. (5) I also hereby state the we have adequate medical and dental insurance coverage and will not, under any circumstances, hold the staff, volunteers, facility, or Tune Up Sports liable for any injuries incurred during training (and future sessions if applicable).
